Registered Nurse (RN) – Hospital Care at Home Program

FT Shift: Day/Evening Rotation (Hours: 7 am – 11 pm/rotation of 12s and 8s) Every 3rd weekend

Location: Hybrid: Remote/In-person

ChristianaCare Hospital Care at Home Program is hiring a Full-time RN to provide care to med-surg patients both virtually and in person.

The RN cares for patients within the scope of the nursing practice license. 

Leveraging a variety of technology tools and software, the RN is responsible for the clinical monitoring and care for the patient including, but not limited to:  clinical assessment, evaluation of clinical information that is being monitored and collected from patients, managing clinical protocols, assessing psychosocial needs, recommending services for the patient to the provider (i.e home health, DME, rehab services), creating collaborative care plans, performing case management functions and escalating clinical concerns to a provider under established protocols.

Our Hospital Care at Home Program enables patients to receive 24/7 hospital-level care from the comfort of their homes rather than in a hospital.  Our team of experts will treat and monitor patient health with the highest level of care. Even though patients are not physically within the hospital's walls, they are constantly connected to healthcare providers. Patients connect with their care team through a combination of:

  • (1) In-person
  • (2) Virtual Visits
  • (3) Monitoring through our Telehealth Platform.

Our Hospital Care at Home services include:

  • A care team comprises physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
  • Laboratory, infusion therapy, wound care, imaging, PT, OT, and EKG services.
  • Advanced practice provider in-home visit.
  • Virtual specialty provider consults.
  • Medication delivery services.

Education & Experience:

  • RN License in Delaware or Compact State
  • BSN preferred or committed to acquiring BSN within 3 years of the hire date.
  • At least two years of med/surg experience as an RN is required.
  • BLS required.

PHYSICAL DEMANDS:

Ability to stand, walk, sit, bend, lift, push, pull, carry, reach, twist, handle, stoop, bend, and climb as per practice area-based Physical Demands Checklist.

WORKING CONDITIONS:

Frequent exposure to biological elements with occasional exposure to chemical and environmental elements as per practice area-based Physical Demands Checklist.
