Pay Range: $38.00–$47.50/hour (Final offer based on your unique skills, experience, and professional background)

What You’ll Do

  • Initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points
  • Initiating the plan of care and revising as necessary
  • Providing services and / or treatments requiring substantial and specialized nursing skill
  • Counseling the client and family members in meeting nursing and related needs
  • Initiating appropriate preventive and rehabilitative nursing procedures
  • Preparing clinical progress notes
  • Provide coordination of services
  • Make referral to other services as needed
  • Informing physicians and other team members of changes in clients’ needs
  • Evaluating outcome of care
  • Supervising LPNs and CNAs
  • Initiating and updating CNA care plans
  • Participating in orientation, onboarding, and education programs for new team members
  • Planning for the clients discharge from services
  • Adhere to all established Standard of Nursing Practices, myPotential at Home, and NLCS Policies & Procedures
  • Ensures continuity and quality care of assigned clients
  • Within the tour of duty accurately assesses clients’ physical and psychosocial status and environmental needs of assigned clients. Identifies needs for other services/disciplines for clients and intervenes appropriately to secure these services. Documents admissions and completes ongoing nursing assessments on assigned clients
  • Collaborates and communicates, with tact and sensitivity, with other team members and departments to coordinate an interdisciplinary approach to client care. Identifies and interprets the psychological reaction of the clients and takes appropriate action within the time frame necessary. Documents nursing interventions and client response to care accurately, timely and completely
  • Performs other duties as assigned

What We’re Looking For

  • Degree in Nursing required
  • Registered Nursing licensed in the State practicing
  • Minimum of 2 years of experience in a hospital, long-term care facility or other related healthcare facility
  • Minimum 1 year of home health experience
  • Must possess strong written and oral communication skills; and ability to maintain confidential information
  • Must have and maintain a valid driver’s license
  • Must have and maintain current CPR
  • Must be proficient in Microsoft Office
